By submitting this form, I am providing my digital signature, agreeing to the terms and conditions set forth by LG Dog Training. I understand and consent to being contacted via email, telephone, and/or text message regarding services, using automated technology or pre-recorded messages. I acknowledge that this consent does not obligate me to purchase services or goods, and that calls may be recorded for quality assurance purposes.

I acknowledge the inherent risks associated with activities such as training, boarding, socializing, walking, and working with dogs. I assume full responsibility for myself and my dog(s) during these activities, understanding that LG Dog Training, Louis Galvis, affiliates, or property owners will not be held responsible for any injuries or incidents that may occur to myself or my dog(s).

I understand that my participation in these activities entails risks such as cuts, scratches, tripping, falling, or other potential accidents. I confirm that my dog(s) are in good health and have received necessary vaccinations as recommended by my veterinarian.

In the event of injury or illness to my dog(s), I authorize LG Dog Training to contact my designated veterinarian. If my veterinarian is unavailable, LG Dog Training has the right to seek immediate care at the nearest animal hospital, for which I agree to reimburse any medical expenses incurred.

I release LG Dog Training, Louis Galvis, affiliates, and property owners from any liability, claims, suits, actions, losses, injuries, or damages arising from my participation or my dog(s) participation in their programs. I indemnify LG Dog Training, Louis Galvis, affiliates, and property owners for any such liabilities or damages.

I acknowledge that I am financially responsible for any damages caused by my dog(s) during training, boarding, socializing, or walking. I grant permission for any media captured during these activities to be used, posted, and advertised by LG Dog Training.

I agree to adhere to the payment terms and conditions outlined in email correspondence and on LG Dog Training's website. I certify that I have read and understood the rules and regulations outlined in this agreement, and I confirm the accuracy of the information provided in this release form.
